How does one improve their Brew ability?
I don't believe I've failed at it once.
Is it just a matter of repetitive use then?

Purchase fat or kill and skin gurth, and/or kill and skin anything that gets you entrails. Craft the entrails into a bunch of useless organs plus some fat.
The only way to max your brew skill is by failing at making soap. Don't shoot the messenger, I didn't code this broke as fuck skill.
Tips for getting brew to max:
1) Start with just the fat and try to make the simple variant.
2) Upon reaching apprentice, go ahead and start combining the fat with citron leaves. The difficulty will increase, thus allowing you to boost your skill even further.
3) Once you reach journeyman or even advanced, add in blood red blossoms to make perfect spheres of soap.
Remember that you only need one fail approximately every hour. Your timers do depend on your wisdom, but from what I could gather in my years of playing 1 hour is a good general estimate.
Follow my advice and you'll max this booger of a skill in no time. Good luck!

As far as I know any brewing using the actual brew command (making tablets, poisons, etc) cannot fail. I'm not even sure what the point of levelling up brew is, unless this has changed.

Is there even a point to improving brew? At one time, you needed to max out brew to branch trapping for Assassins. But trapping skill is taken out of the game.

Zul wanted to master brew in order to develop new warpaint for the Soh. Upon mastering and writing up the new item, he was told that "oops, we forgot to inform players that you cannot mastercraft any brewed item.
So no, there is no point to master brew other than to create interesting forms of soap.
